The idiom comes from the Yuan dynasty anonymous play 'Selling Grain in Chenzhou' (陈州粜米), which contains the line 'Having presented a plan for peace at the palace, he alone occupied the turtle head as number one.' In the imperial examination era, the palace steps were carved with the image of Ao, a mythical giant turtle or sea monster. When the examination results were announced, the top-ranking scholar, known as the zhuangyuan, would stand on the carved Ao head to receive the proclamation. Over time, 'occupying the Ao head alone' became a metaphor for ranking first in any endeavor.
